Request for Facilities Fee Waiver*

Please fill out this form completely (all fields are required) to request that basic facility fees be waived. Deposits are required for all reservations and are fully refundable if conditions agreed upon at the time of rental are upheld. Fees such as sound system rental, social hall security, and other fees not directly involved in the renting of facilities are not available to be waived. By submitting this form you are requesting a waiver of the facilities fee with the understanding that, as a representative of McMurry University, the facilities that are used will be left in the same condition in which they were rented and all rules and regulations stated in the rental policies will be adhered to.
